Adriana Anaya Saavedra,
Bilingual, Spanish, Lead Care Manager

I previously worked as an Eligibility Specialist for Colusa County DHHS, a Billing Manager at a non-profit behavioral health agency in Yolo County, an Office Manager at a private Practice Psychiatrist in Sacramento. I realized my passion is to work directly with the public to provide services, resources and education to those facing challenges. I love meeting people of all backgrounds and getting to know them. I also enjoy outreach and networking. I have two daughters that I spoil despite saying I wouldn’t. We enjoy mini trips and food on the weekends and holidays. I wish I could say I still have many hobbies but at the moment sleeping and being a mom are it. I am excited and proud to have joined TCCAP.

Community Health Workers
Community Health Workers (CHWs) are trusted members of the community who play a vital role in connecting individuals and families to the care and support they need. CHWs help bridge gaps between health and social services by offering hands-on assistance, education, and advocacy—especially for those who may face barriers due to language, culture, or access.
Our CHWs provide a wide range of services, including:
-
Helping clients navigate healthcare systems and understand care plans
-
Connecting individuals to community resources such as housing, food, and transportation
-
Providing education on chronic disease management, healthy living, and preventive care
-
Offering emotional support and motivational coaching to help clients reach their wellness goals
-
Conducting home visits and outreach to meet people where they are
-
Assisting with paperwork and applications for health coverage and social services
At the heart of it all, CHWs are here to build trusting relationships and empower community members to take charge of their health and well-being. Their lived experience and deep understanding of the communities they serve make them a powerful force for change and connection.
