Enhanced Care Management (ECM) is a free service for Medi-Cal members with complex health and social needs. It helps connect you to the right care and support to improve your health and well-being
Who Can Get ECM?

You may qualify for ECM if you:

Are experiencing homelessness.

Have frequent hospital or emergency room visits.

Have serious mental health or substance use challenges.
-
Are leaving jail, prison, or a youth correctional facility.

Live in the community but need help to avoid a nursing home.

Are a child or youth with special health needs.
-
Are pregnant or just had a baby and need extra care.

How Does ECM Help?
ECM connects you with a Care Manager who works with you and your family to:

Create a personalized care plan.

Help you access doctors, mental health support, and other services.

Coordinate housing, transportation, or food assistance if needed.
