Community Health Assessment

Tri Counties Community Action Partnership (TCCAP) plays a significant role in community health assessments for Glenn and Colusa counties. The organization is dedicated to addressing the health and wellness needs of these communities through collaboration and the implementation of evidence-based, community-driven strategies. TCCAP partnered closely with Glenn County Public Health to facilitate the CHA process.  TCCAP gives credit to Glenn County Public health’s methodology as a model for the Colusa County Community Health Assessment, in which TCCAP took the lead. The process focused on identifying and assessing local health challenges, leveraging data to guide decisions, and working with partners to implement programs aimed at improving overall health outcomes. These initiatives often center around vulnerable populations, including low-income individuals, and emphasize holistic approaches that encompass behavioral health, social determinants of health, and equitable access to resources. TCCAP's work is vital in driving systemic improvements and addressing the unique needs of these counties.
