PROGRAM COORDINATOR

The Program Coordinator (CalAIM Supervisor) provides leadership and day-to-day support for Community Health Workers and Lead Care Managers delivering Enhanced Care Management services. This role combines strengths-based supervision, program coordination, and collaboration with clinical leadership to ensure high-quality, compliant, and relationship-centered care. Program Coordinators help build effective teams, support staff growth, and work closely with partners to improve access to services and outcomes for community members.

The CHW Supervisor / Program Manager oversees Community Health Workers and supports the development and delivery of community-based programs. This role provides strengths-based supervision, program coordination, and guidance to ensure services are relationship-centered, trauma-informed, and responsive to community needs. The CHW Supervisor / Program Manager works closely with staff and partners to strengthen outreach, build trust, and support meaningful connections between community members and services.

Lead Care Managers work directly in the community, partnering with members to identify goals, build stability, and access coordinated supports that promote long-term health and wellbeing. This position is ideal for someone who values trust, consistency, and meeting people where they are while working within structured program requirements.

Community Health Worker

Community Health Workers (CHWs) work with community members to determine needs for services/resources and provide case management and crisis intervention across various program areas with a focus of collaborating closely with a variety of providers of services and resources.
