At Tri Counties Community Action Partnership, we understand that transportation can be a barrier to accessing the care and resources you need. That’s why we’re here to help. Our transportation program provides reliable rides for individuals and families facing challenges in getting to health appointments and accessing services for health-related needs.
Who We Serve

Available Counties: Glenn and Colusa counties. (Trinity starting in March)

Eligibility: We have grant funding in Colusa County that enables us to help anyone in the community through June 2026. We are contracted with Partnership Health Plan (Medi-Cal) to provide transportation for all other rides.

What We Offer

Health Appointments: Rides to and from doctor’s visits, therapy sessions, and more.

Health-Related Social Needs: Access to essential services such as pharmacies, counseling, and nutrition programs.

Support: Our friendly Community Health Workers can assist with scheduling and navigating Medi-Cal transportation benefits.​

YOU ARE NOT GUARANTEED TO HAVE US AS YOUR PROVIDER. You may ask for Tri Counties Community Action Partnership to be your preferred provider.

Schedule a Ride: Call us at 1 (530) 433-4420 ext. 2 at least 48 hours before your appointment.

Cancellations: Please notify us at least 24 hours in advance if you need to cancel.
