At Tri Counties Community Action Partnership, we understand that transportation can be a barrier to accessing the care and resources you need. That’s why we’re here to help. Our transportation program provides reliable rides for individuals and families facing challenges in getting to health appointments and accessing services for health-related needs.

Support: Our friendly Community Health Workers can assist with scheduling and navigating Medi-Cal transportation benefits.
